THE SHORT ANSWER: Ferritin is the protein that stores iron, and a serum ferritin test is the most common non-invasive way to estimate how much iron the body has banked. It matters because a standard complete blood count does not measure iron stores at all. A CBC measures hemoglobin and red cell size. Someone can run down their iron reserves for months or years while hemoglobin still lands inside the reference range, which is why a person can feel exhausted, cold, and short of breath on a set of stairs and still be told their bloodwork looks fine. Ferritin also rises with inflammation, so it gets read alongside a marker like CRP rather than on its own. Ferritin is not a diagnosis and it is not a treatment plan. It is one number, ordered and interpreted by a licensed provider, that helps explain what a CBC leaves out.
Why does a "normal" CBC miss low iron?
A complete blood count is a good test at the job it was built for. It counts red cells, measures hemoglobin, and reports how big those cells are. What it does not do is tell you how much iron is sitting in storage.
The body protects circulating hemoglobin first. When intake falls short of losses, iron comes out of storage in the liver, spleen, and bone marrow to keep red cell production going. That process can run a long time before hemoglobin drops far enough to flag anemia. Hematology literature has a name for the gap: iron deficiency without anemia. Stores are depleted, hemoglobin is still in range, and the CBC reads clean.
That is the whole reason ferritin exists as a separate order. If nobody asks for it, nobody sees it.
There is a second wrinkle. Ferritin is an acute phase reactant, which means infection, inflammation, obesity, liver disease, and heavy training can all push the number up independently of how much iron is actually stored. A ferritin that looks reassuring in the presence of active inflammation can be masking depleted stores underneath. This is why a provider ordering ferritin will often order an inflammatory marker with it, and why a single number pulled out of context is worth very little.
What do ferritin reference ranges actually mean?
Here is where the conversation gets interesting, and where the phrase "your labs are normal" starts to lose some of its authority.
A lab reference range is a statistical description of the population that lab drew from. It is not a definition of optimal, and it was never designed to be. Many commercial panels set the lower bound for women somewhere in the low double digits. The World Health Organization's guidance on ferritin concentrations uses a threshold below 15 micrograms per liter to indicate depleted iron stores in adults, and notes that in the presence of infection or inflammation a higher cutoff applies. Separately, a body of diagnostic-accuracy work in hematology has looked at higher thresholds as more sensitive for catching absent marrow iron, and clinicians disagree about exactly where the line belongs.
Two honest takeaways from that:
First, a result can sit above a lab's printed lower bound and still sit in a range that a hematologist would want to look at again. "In range" and "where you want to be" are not the same statement.
Second, nobody should self-diagnose off a threshold they read in a blog post, including this one. Where the cutoff belongs for a specific person depends on symptoms, inflammation, kidney function, menstrual history, pregnancy status, training load, and medications. That is a clinical judgment, and at Vitality Dartmouth it belongs to our collaborating nurse practitioner or physician, who orders the panel and interprets it. I build the program and coach the training. I do not diagnose and I do not prescribe.
Why does this come up so often for women in midlife?
Because the arithmetic changes.
Menstrual blood loss is the most common driver of iron depletion in premenopausal women, and perimenopause frequently brings heavier and less predictable cycles rather than lighter ones. The intuition that periods are "winding down" can run in the opposite direction of what is actually happening to iron losses in a given year.
Layer in the other inputs. Dietary iron intake often drops when someone cuts calories to manage weight. Plant-source non-heme iron is absorbed less efficiently than heme iron from animal sources. Coffee and tea with meals reduce absorption. Some acid-reducing medications reduce it further. Gastrointestinal blood loss is a separate and important cause at any age and one that needs a provider's evaluation rather than a supplement.
Iron is not only about red cells, either. It is a component of myoglobin in muscle and a cofactor in mitochondrial enzymes that handle energy production. That is the mechanistic reason low iron status shows up as reduced exercise capacity and slower recovery before it ever shows up as anemia, and it is why this marker matters to the same person who is trying to add strength training to a weight-management program and cannot understand why the sessions feel so much harder than they should.
Should everyone just take an iron supplement?
No. This is the part that gets skipped, and it is the part with real downside.
Iron is one of the few nutrients where more is not neutral. The body has no efficient route to excrete excess iron, so what goes in largely stays in. Supplementing without testing risks loading iron into someone who does not need it, and it papers over causes of blood loss that need to be found rather than offset.
There is also a genetic layer. Hereditary hemochromatosis, most commonly associated with variants in the HFE gene, causes the gut to absorb more dietary iron than it should, and iron accumulates in the liver, heart, and pancreas over years. It is one of the more common inherited conditions in people of Northern European ancestry, which is relevant on the SouthCoast. Someone carrying that genotype and self-prescribing iron because they feel tired is adding to a problem instead of solving one. This is exactly the kind of question our genomic and lab testing is built to surface before a supplement decision gets made, and it is why testing comes before supplements rather than after.
Even when iron repletion is appropriate, the how matters and it belongs with a prescriber. Research published in Lancet Haematology examined dosing intervals and found that giving iron on alternate days rather than daily improved fractional absorption in iron-depleted women, a finding tied to the hormone hepcidin, which rises after a dose and blunts absorption of the next one. That is a useful piece of evidence and it is still not a self-serve instruction. Dose, form, interval, and duration are prescribing decisions, and follow-up testing is how anyone knows whether the plan worked.
What does screening guidance say?
Less than most people assume, which is worth knowing.
The U.S. Preventive Services Task Force has addressed screening for iron deficiency and iron deficiency anemia in pregnancy and concluded the current evidence is insufficient to assess the balance of benefits and harms, an I statement rather than a recommendation for or against. There is no broad national recommendation that every non-pregnant adult woman get a routine ferritin drawn at her physical.
That is not a scandal. Task force recommendations are built around population screening evidence, and the absence of a recommendation is a statement about the evidence base, not a statement that the marker is meaningless for a symptomatic person. It does explain something practical, though. If a standard annual physical does not include ferritin, and the guidance does not push for it, the number tends not to get drawn unless somebody specifically asks for it.
That is the case for asking.
